We do require deposits at the time of scheduling for office consultations, dental and soft tissue procedures. Please see the clinic policies page under the deposits section for more information.
When requesting an appointment online, if you have not heard from us within 24 hours please call the clinic to verify your appointment.
For your first visit to our clinic, we will have you fill out a “new client” form. Please refer to the new client page for instructions. Please note that we do require a $50.00 deposit at the time of scheduling your appointment. This deposit will be put towards your appointment. However, if you are to no-show or cancel an hour or later prior to your scheduled time, the deposit may not be re-used for future appointments or re-funded.
