Welcome to the Petcetera Animal Clinic family! Before your first visit to the clinic, we will have you fill out a “New Client” information form. Please email it to [email protected] at least 24 hours prior to your appointment.
If your pet has received medical care elsewhere, please either attach the records to your emailed New Client Form or bring it with you to your appointment.

This is an example of the consent form that is required before all anesthetic/sedation procedures. All owners are advised to read through this form before any anesthetic procedure in order to fully understand and consent to services that are being performed.
